Patient Handoff In Perinatal Patient Safety: Communication Techniques Used

Perinatal patient safety forms the most important part of the optimal health care of the mother and the neonate. Perinatal period which commences at 20 completed weeks of gestation and ends seven completed days after birth, is a risk period for the pregnant, fetus and puerperal. Perinatal patient safety aims at ensuring that the neonate and the mother receive care at gestation, throughout and after the labor in an environment as safest as possible, as well as at preventing maternal and neonatal mortality. Nurses and midwifes are no doubt the most important members of the team in ensuring perinatal patient safety.
Studies conducted toward the root causes of medical errors report that medical errors results substantially from miscommunication. Complete and accurate information that healthcare personnel conveys to each other is of vital importance during patient handoff process.
Communication is one of the most important measures taken in order to prevent harm to the patient during the provision of health care services for a complete, precise and explicit handoff of the patient, which is of vital importance in terms of perinatal patient safety.
Some professional communication techniques have been developed which involve questioning and answering opportunities with a view to ensure a standardized communication during patient handoff process. SBAR, I PASS THE BATON, 5 P’S, HANDOFFS are the most frequently used professional communication techniques.
This article aims at introducing professional communication techniques in the process of patient handoff and laying emphasis on their use in the field of perinatology as well as increasing the awareness in this respect.

Perinatal Hasta Güvenliğinde Hasta Teslimi: Kullanılan İletişim Teknikleri

Perinatal hasta güvenliği anne ve bebeğin optimal düzeydeki sağlık bakımının en önemli parçasını oluşturmaktadır. Gebeliğin 20. haftasında başlayan ve yenidoğan yaşamının ilk haftasına kadar geçen süreyi kapsayan perinatal dönem, gebe, fetüs ve lohusa için riskli bir dönemdir. Perinatal hasta güvenliğinde amaç, bebek ve annenin gebelikte, doğum eylemi boyunca ve sonrasında mümkün olan en güvenli çevrede bakım almasını sağlamak, anne ve bebek ölümlerini önlemektedir. Perinatal hasta güvenliğinin sağlanmasında ekibin en önemli üyeleri kuşkusuz hemşire ve ebelerdir.
Tıbbi hataların kök nedenlerine yönelik olarak yapılan çalışmalarda iletişimin büyük oranda tıbbi hataların kaynağı olduğu bildirilmektedir. Sağlık çalışanlarının özellikle hasta teslim sürecinde birbirlerine aktardığı bilginin tam ve eksiksiz olması hasta teslim sürecinde hayati bir önem taşımaktadır.
Perinatal hasta güvenliği açısından hayati bir önem taşıyan hasta tesliminin eksiksiz, tam ve açık bir şekilde iletilmesinde iletişimin sağlık bakım hizmetlerinin sunumu aşamasında, hastaya zarar verilmesini önlemek amacıyla alınan en önemli önlemlerden biridir.
Hasta teslim sürecinde standardize bir iletişim sağlamak amacıyla soru sorma ve cevaplama fırsatlarını içeren profesyonel iletişim teknikleri geliştirilmiştir. Bunlar arasında en sık kullanılanlar; SBAR, I Pass The Baton, 5 P’s, Handoffs profesyonel iletişim teknikleridir.
Bu makalede; hasta teslim sürecinde profesyonel iletişim tekniklerini tanıtmak ve perinatoloji alanında kullanımının önemini vurgulamak, bu konuda farkındalığı artırmak amaçlanmıştır.
