The role of midwife and nurse in pregnancy care with addison's disease

Addison’s Disease is a chronic disease that occurs with destruction of surrenal glands. Adrenal diseases are critical in the sense that they may cause serious maternal morbidity and mortality during pregnancy. Diagnosis of Addison's disease is difficult in pregnancy because its symptoms are very much similar to physiological signs of pregnancy; which may retard the diagnosis. Besides, detection of the disease with diagnosis is crucial in fetal and maternal risks. Fetuses of pregnant women with Addison’s Disease may face the risk of adrenal insufficiency, preterm birth, abortion, low birth weight and intrauterine growth retardation. Addison’s Disease crisis may occur due to stressful situations, progression of Addison’s Disease, infectious diseases, excessive vomiting and acute hypotension. Therefore; increasing treatment doses as well as midwifery and nursing care that will prevent these conditions should not be ignored in order to prevent development of Addison’s Disease. As a result; although addison disease in pregnancy is not widespread; it may lead to critical problems due to its high risk and therefore this review was done in order to guide midwives and nurses while giving care to the pregnant women.

Addison hastalığı olan gebenin bakımında ebe ve hemşirenin rolü

Addison Hastalığı sürrenal bezlerin destrüksiyonu ile giden kronik bir hastalıktır. Adrenal hastalıklar gebelikte maternal ve fetal ciddi morbidite ve mortalite nedeni olabilmeleri nedeniyle önemlidir. Addison hastalığına tanı koymak gebelikte zordur çünkü bu hastalıktaki semptomlar gebeliğin fizyolojik belirtileri ile büyük oranda benzemektedir bu nedenle tanı konması gecikebilir. Ayırıcı tanı ile hastalığın saptanması fetal ve maternal riskler açısından önemlidir. Addison hastalığı olan gebelerin fetüslerinde adrenal yetmezlik, erken doğum, abortus, düşük doğum ağırlığı ve intauterin gelişme geriliği riski mevcuttur. Yaşanan stresli durumlar, addison hastalığının ilerlemesi, enfeksiyon hastalığı, aşırı kusma ve akut hipotansiyon nedeniyle addison krizi gelişebilmektedir. Bu nedenle yaşanan bu durumlarda addison krizi gelişimini engellemek için tedavi rejimindeki değişikliklerin yanı sıra bu durumların görülmesini engelleyici ebelik ve hemşirelik bakımı göz ardı edilmemelidir. Sonuç olarak, gebelikte addison hastalığı yaygın görülmemesine rağmen taşıdığı risk açısından büyük sorunlara yol açabileceği için bu derleme gebelere verilecek olan bakımda ebe ve hemşirelere rehber olabilmek amacıyla yazılmıştır.
