Hopelessness Level and the Affecting Factors among Patients after Total Hip Arthroplasty

INTRODUCTION: Among patients who undergo total hip arthroplasty, problems such as experiencing pain in the postoperative period, the occurrence of complications due to immobilization and the need for an assistive device may cause hopelessness. The patients may feel himself / herself inadequate and unhappy. The aim of this study is to determine hopelessness level and affecting the factors among patients who undergo total hip arthroplasty.
METHODS: The descriptive study was carried out between May 2016 and September 2018 with the participation of 90 patients who undergo total hip arthroplasty in a university hospital. Institution and ethics committee permission was obtained prior to the study. “Patient Descriptive Form” and “Beck Hopelessness Scale” were used to collect the data. Data were analyzed by using the Mann Whitney U, the Chi square, the Kruskal Wallis H and the Spearman correlation tests in SPSS 20.0 package program.
RESULTS: Seventy-two point two percent of the patients were female and the mean age was 64.4 ± 12.4 years. The mean total Beck Hopelessness Scale score of the patients was 6.66 ± 4.93. The mean scores of the scale were higher in patients with total hip arthroplasty compared to the patients with primary total hip arthroplasty (p < 0.05). There was a positive correlation between age and hopelessness (p < 0.05).
DISCUSSION AND CONCLUSION: The hopelessness level of patients who undergo total hip arthroplasty was found to be “mild level”. In the postoperative period, we think that the negative effects of hopelessness on the patient should be reduced by determining the risky patients in terms of hopelessness and should be provide appropriate nursing care by the surgical nurses.

Total Kalça Protezi Ameliyatı Sonrası Hastalarda Umutsuzluk Düzeyi ve Etkileyen Faktörler

GİRİŞ ve AMAÇ: Total kalça protezi uygulanan hastalarda ameliyat sonrası dönemde ağrı yaşanması, immobilizasyona bağlı komplikasyonların gelişmesi ve yardımcı araç gerece gereksinim duyulması gibi sorunlar umutsuzluğa yol açabilmektedir. Hastalar bu durumda kendini yetersiz ve mutsuz hissedebilir. Bu çalışmanın amacı total kalça protezi uygulanan hastalarda umutsuzluk düzeyi ve etkileyen faktörleri belirlemektir.
YÖNTEM ve GEREÇLER: Tanımlayıcı tipteki bu çalışma Mayıs 2016–Eylül 2018 tarihleri arasında bir üniversite hastanesinde TKP uygulanan 90 hastanın katılımıyla gerçekleştirilmiştir. Çalışma öncesinde kurum ve etik kurul izni alınmıştır. Hastalardan bilgilendirilmiş gönüllü olurları alındı. Veri toplamada “Hasta Tanım Formu” ve “Beck Umutsuzluk Ölçeği” kullanılmıştır. Veriler SPSS 20.0 programında Mann Whitney U testi, Ki kare, Spearman Korelasyon Analizi ve Kruskal Wallis H testleri kullanılarak analiz edilmiştir.
BULGULAR: Hastaların %72,2’sinin (n=65) kadın ve yaş ortalamasının 64.4±12.4 olduğu saptanmıştır. Hastaların umutsuzluk ölçeği toplam puan ortalaması 6.66±4.93 bulunmuştur. Total kalça protezi uygulanan hastaların umutsuzluk ölçeği puan ortalamaları primer total kalça protezi uygulanan hastalara oranla yüksek bulunmuştur (p<0.05). Yaş ile umutsuzluk düzeyi arasında pozitif korelasyon belirlenmiştir (p<0.05).
TARTIŞMA ve SONUÇ: Total kalça protezi uygulanan hastalarda umutsuzluk “hafif düzeyde” bulunmuştur. Ameliyat sonrası dönemde cerrahi hemşirelerinin umutsuzluk açısından riskli hastaları belirlemesi ve uygun hemşirelik bakımı sunması ile umutsuzluğun hasta üzerindeki olumsuz etkilerin azaltılabileceği düşünülmektedir.
